These Terms & Conditions ("Terms") govern your access to and use of slimmeds.shop and the telehealth services provided by Slim Meds Telehealth ("Slim Meds," "we," "us," or "our"). By using the Site, submitting our intake form, or receiving services, you agree to these Terms. If you do not agree, do not use the Site or Services.

1. Eligibility

You must be at least 18 years of age and a resident of a U.S. state where our affiliated medical providers are licensed to provide telehealth services. By using the Services you represent and warrant that you meet these requirements and that all information you provide is true, accurate, and complete.

2. Telehealth Services & The Provider Relationship

Slim Meds is a technology platform. Slim Meds is not a medical practice and does not provide medical advice, diagnosis, or treatment. Medical services, including evaluations and prescriptions, are provided by independent, licensed healthcare providers ("Providers") who use the platform to deliver care. A clinician-patient relationship is established only after a Provider accepts you as a patient following a clinical evaluation. The Providers — not Slim Meds — are solely responsible for the medical services they render.

3. Not an Emergency Service

Slim Meds is not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use the Site to communicate emergency or time-sensitive medical concerns.

4. Compounded Medications

Compounded medications dispensed through the platform are prepared by state-licensed compounding pharmacies pursuant to a valid prescription. Compounded drugs are not FDA-approved as finished drug products and the FDA does not verify their safety, quality, or effectiveness in the same manner as commercially manufactured drugs. Your Provider will discuss the risks, benefits, and alternatives before prescribing.

5. No Guarantee of Treatment or Results

Submission of intake information does not guarantee a prescription, treatment plan, or any specific outcome. Your Provider may decline to prescribe based on clinical judgment. Weight-loss results vary significantly; testimonials reflect individual experiences and are not typical results.

6. Subscription, Billing & Cancellation

Treatment plans are sold on a recurring subscription basis (typically monthly or quarterly). You authorize us to charge your payment method automatically on each renewal date until you cancel. You may cancel at any time from your account portal or by emailing care@slimmeds.shop; cancellation stops future shipments and charges but does not entitle you to a refund of medications already shipped. All sales of compounded medication, once shipped, are final and non-refundable except where required by law.

7. Communications & TCPA Consent

By providing your phone number and checking the consent box on our intake form, you provide your express written consent, under the Telephone Consumer Protection Act (TCPA) and applicable state law, for Slim Meds and its Providers and authorized service partners to contact you at that number — including via autodialed and pre-recorded calls and SMS/MMS text messages — for marketing, transactional, care-related, appointment, billing, and shipping purposes. Consent is not a condition of purchase. Message and data rates may apply. Message frequency varies.

You may opt out of marketing texts at any time by replying STOP to any message; reply HELP for help. You may opt out of marketing emails using the unsubscribe link in each email. You may continue to receive transactional messages (e.g., shipping notifications, refill reminders, clinical messages from your Provider) that are necessary to provide the Services even after opting out of marketing.

8. Acceptable Use

You agree not to: (a) provide false or misleading information; (b) impersonate any person; (c) share your account with anyone; (d) resell, divert, or distribute any medication dispensed to you; (e) use the Services for any unlawful purpose; or (f) attempt to access, scrape, or interfere with the Site's systems or other users' data.

9. Intellectual Property

All content on the Site — including text, graphics, logos, photos, and software — is owned by or licensed to Slim Meds and is protected by copyright, trademark, and other laws. You may not copy, modify, distribute, or create derivative works without our prior written consent.

10. Disclaimer of Warranties

THE SITE AND SERVICES ARE PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. We do not warrant that the Services will be uninterrupted, error-free, or secure.

11. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, SLIM MEDS AND ITS OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS, DATA, OR GOODWILL, ARISING OUT OF OR RELATED TO YOUR USE OF THE SITE OR SERVICES. Our total aggregate liability shall not exceed the amount you paid to Slim Meds in the three (3) months preceding the claim.

12. Indemnification

You agree to indemnify and hold harmless Slim Meds and its affiliates from any claim, loss, liability, or expense (including reasonable attorneys' fees) arising from your breach of these Terms, your misuse of the Services, or your violation of any law or third-party right.

13. Arbitration & Class-Action Waiver

Any dispute arising from these Terms or the Services shall be resolved exclusively by final and binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, seated in Wilmington, Delaware. You and Slim Meds each waive the right to a jury trial and to participate in any class, collective, or representative action. You may opt out of this arbitration provision by sending written notice to legal@slimmeds.shop within 30 days of first accepting these Terms.

14. Governing Law

These Terms are governed by the laws of the State of Delaware, excluding its conflict-of-laws rules, and by applicable U.S. federal law.

15. Changes to These Terms

We may update these Terms from time to time. Material changes will be posted on this page with a new "Last updated" date and, where required, notified to you by email or in-app notice. Your continued use of the Services after the effective date constitutes acceptance of the updated Terms.

Identifiers — name, email, phone, mailing/shipping address, date of birth.
Health information — height, weight, medical history, medications, allergies, intake responses, photographs, lab results, prescription history, and provider notes (collectively, "PHI").
Payment information — billing address and the last 4 digits of your card (full card data is handled by our PCI-compliant payment processor; we do not store it).
Device & usage data — IP address, device type, browser, pages viewed, referring URL, cookies, and analytics events.
Communications — the content of messages you send us, including chat, email, and SMS.

2. How We Use Your Information

We use your information to: (a) determine your eligibility for telehealth services; (b) facilitate clinical evaluation, prescription, and dispensing; (c) process payments and ship medications; (d) communicate with you about your care, account, and orders; (e) send marketing communications you have consented to receive; (f) operate, secure, and improve the Site; (g) comply with legal, regulatory, and safety obligations.

3. HIPAA Notice of Privacy Practices

The Providers using our platform are Covered Entities under HIPAA. Slim Meds acts as a Business Associate of the Providers with respect to PHI. PHI is used and disclosed only for treatment, payment, and healthcare operations, or as otherwise permitted by HIPAA or with your written authorization. We maintain administrative, physical, and technical safeguards — including encryption in transit and at rest, role-based access controls, and routine audits — to protect PHI.

4. How We Share Information

We do not sell your PHI. We share information only:

With licensed Providers and pharmacies who deliver your care and dispense your medication.
With service providers (e.g., hosting, analytics, payment processing, shipping, SMS/email vendors) who are contractually required to protect your information and use it only on our behalf.
With legal and regulatory authorities when required by law, subpoena, court order, or to protect the safety of any person.
In connection with a merger, acquisition, or sale of assets, subject to confidentiality protections.
With your explicit consent in any other case.

5. SMS & Email Communications (TCPA / CAN-SPAM)

If you provide your phone number and check the consent box on our intake form, you give your express written consent to receive autodialed and pre-recorded calls and SMS/MMS text messages from Slim Meds and our Providers at that number, for marketing, transactional, and care-related purposes. Consent is not a condition of purchase. Message and data rates may apply; message frequency varies. Reply STOP to opt out of marketing texts, HELP for help. You may unsubscribe from marketing emails at any time using the unsubscribe link or by emailing privacy@slimmeds.shop. Transactional and clinically necessary messages may continue after marketing opt-out.

We do not share mobile opt-in data with third parties for their own marketing purposes.

6. Cookies & Tracking

We use cookies and similar technologies to operate the Site, remember preferences, measure performance, and (with consent where required) deliver advertising. You can control cookies through your browser settings. We honor the Global Privacy Control (GPC) signal as a valid opt-out of sale/sharing for targeted advertising under applicable state laws. We do not use PHI for advertising.

7. Your Privacy Rights

8. Data Retention

We retain personal information for as long as your account is active and for the period required by law (e.g., medical records are retained for the minimum period required by the state in which your Provider is licensed, typically 7 to 10 years). After the retention period, we securely delete or de-identify the information.

9. Security

We use industry-standard administrative, technical, and physical safeguards, including TLS encryption in transit, AES-256 encryption at rest, multi-factor authentication, intrusion monitoring, and least-privilege access controls. No method of transmission or storage is 100% secure, but we work continuously to protect your data and will notify affected individuals of any breach in accordance with applicable law.

10. Children

The Site and Services are not directed to individuals under 18. We do not knowingly collect personal information from children. If you believe a child has provided information to us, contact privacy@slimmeds.shop and we will promptly delete it.

11. Out-of-State & International Users

The Services are offered only to U.S. residents in states where our Providers are licensed. If you access the Site from outside the United States, you do so at your own risk and your information will be processed in the United States.
